Carbs and insulin make you fat

The claim, as heard: "Fat gain is caused by insulin, not by calories. Cut the carbs and the weight takes care of itself — calories don't matter."

What the data show. This one deserves respect, because behind the slogan is a real scientific debate between two models of obesity — one in which the energy balance is primary and appetite and food environment drive intake, and one in which carbohydrate-driven insulin directs energy into fat cells and causes the hunger. Both are argued by serious researchers in paired position papers, and this page cites both. The test is what happens when the models make different predictions. They do: the carbohydrate–insulin model predicts that a lower-carbohydrate diet should lose more weight at equal calories. In the two largest controlled comparisons — one assigning healthy low-carbohydrate or healthy low-fat diets for a year, one assigning four macronutrient patterns for two years — weight loss did not differ meaningfully by carbohydrate content. The Cochrane review of low-carbohydrate versus balanced-carbohydrate diets reaches the same place: little to no difference in weight at up to two years. Those trials had their limits — intensive support in one, poor adherence to the target macros in the other, high dropout in many — which is why the debate continues. But on the specific question of weight, the dominant evidence is against the claim.

Verdict: the slogan is false; the underlying debate is real and unresolved on mechanism. Evidence grade A for the weight question, from two large trials and a Cochrane review.

The grain of truth. For some people, lower carbohydrate means easier appetite control and a faster start (water and glycogen), and both help adherence. Insulin does matter for people with diabetes. Neither makes calories irrelevant.

One sentence to remember: a deficit removes fat with carbohydrate high or low; choose the level you can keep, not the one the theory prefers.
